A Medicare Advantage PPO plan with prescription drug coverage:
- Allows members to obtain covered services from any provider they choose, but covers a larger portion of costs when members use in-network providers.

Humana Gold Choice®
A Medicare Advantage Private Fee-for-Service (PFFS) plan with prescription drug coverage:
- No Network! Humana's PFFS plan** gives members the freedom to choose any providers or healthcare facilities that accept Medicare assignment and agrees to bill and accept payment from Humana.
Part D stand-alone plans (PDP)
Prescription drug coverage only:
- Available with Original Medicare or a Medicare supplement plan.
- Three plans to choose from: Standard, Enhanced, and Complete.

±A Medicare approved PPO Special Needs Plans(SNP) available to anyone who meets the specific eligibility requirements of the SNP and is enrolled in both Part A and Part B of Medicare through age or disability. To qualify for a Chronic Disease SNP, physician diagnosis of the disease must be verified.
