A Medicare Advantage PPO plan with prescription drug coverage:
- Allows members to obtain covered services from any provider they choose, but covers a larger portion of costs when members use in-network providers.

Humana Gold Choice®
A Medicare Advantage Private Fee-for-Service (PFFS) plan with prescription drug coverage:
- No Network! Humana's PFFS plan** gives members the freedom to choose any providers or healthcare facilities that accept Medicare assignment and agrees to bill and accept payment from Humana.
Part D† stand-alone plans (PDP)
Prescription drug coverage only:
- Available with Original Medicare or a Medicare supplement plan.
- Three plans to choose from: Standard, Enhanced, and Complete.
The products and services described below are neither offered nor guaranteed under Humana's contract with the Medicare program. In addition, they are not subject to the Medicare appeals process. Any disputes regarding these products and services may be subject to the Humana grievance process:
